Not getting mediclaim from National Health Insurance

Dear sir,
I am a consumer of national health insurance, i have taken a mediclaim policy for my family two year back and i have payed and got renewed this year also.
in last two year company did not provided me TPA card and i made calls several time for that but still after two and half year later i could not get that i might have called them thousand time for that,but this is not only the issue
in the month of march my wife underwent for a major surgical operation in gov hospital in bikaner at that time i talked to DO of company he told me to submit a latter with in 24 hr i made that and then i filed a mediclaim at the end of march 2010
but evenafter five month i didnot got any reply ,the only thing is i have to contact them regularly and they reply me same answer that my file is sent to higher authority  i am hope less now could any one help me in this matter
